We understand that immigration paperwork can feel overwhelming. Therefore, our team works efficiently, explains every step clearly, and ensures your Form I-693 is completed accurately and sealed properly to avoid delays.

If you apply for permanent residency, U.S. immigration law requires a medical exam. A USCIS-approved civil surgeon must perform the exam. The doctor completes Form I-693, seals it, and gives it to you. You must submit the sealed envelope with your application. Do not open it.

Importantly, this is not a full annual physical. Instead, the exam focuses only on conditions that affect immigration eligibility.

The U.S. immigration medical exam requires a physical examination, medical history review, required vaccinations, and testing for tuberculosis, syphilis, and gonorrhea. A USCIS-approved civil surgeon performs the exam, completes Form I-693, and provides it in a sealed envelope for submission with your application.

What the Exam Includes

Physical Examination

The doctor checks your eyes, ears, nose, throat, heart, lungs, abdomen, skin, and lymph nodes. The goal is to identify communicable diseases or serious health concerns.

Mental Health Screening

The doctor briefly reviews your mental health history. They also screen for substance abuse. However, a diagnosis alone does not make someone inadmissible. The key issue is whether the condition involves harmful or unstable behavior.

Required Vaccinations

The doctor reviews your immunization records. You must receive required vaccines such as MMR, Hepatitis B, Varicella, Polio, Tdap, and others based on age guidelines.

Required Laboratory Tests

The exam includes testing for tuberculosis (TB) and syphilis. In certain age groups, screening for gonorrhea is also required.

What to Bring to Your Appointment

To avoid delays, bring the following:

  • Valid passport or government-issued photo ID

  • Vaccination records

  • Medical history, including past surgeries or treatments

  • Records of prior TB or syphilis treatment (if applicable)

  • List of current medications

  • Form I-693 applicant section completed (do not sign)

  • Payment for the exam and any required vaccines or lab tests

Important Things to Know

Validity

In most cases, exam results remain valid for six months. However, certain conditions may shorten that time frame.

Sealed Envelope

The doctor must seal Form I-693 before giving it to you. You must submit it unopened.

Designated Civil Surgeon

Only a USCIS-designated civil surgeon can perform this exam. Exams from non-approved doctors are not accepted.
